Scheduling your first appointment:
When you need to schedule an appointment, our receptionists are here and ready to assist you. If you have any questions before your first visit, please feel free to call us. You’ll find our office numbers and directions here »
When you come for your appointment, please bring the following:
To ensure we deliver the highest level of care, please bring the following information to your first appointment. This will help us create your electronic record and ensure we have all the essential details needed for your treatment.
- Driver’s License or a valid photo ID
- A list with all your medications (if any)
- Insurance information and card
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc., and any other relevant information
- Please arrive 20-30 minutes early to register and complete forms
If you need to cancel an appointment
Telephone the office during business hours and allow at least 24 hours’ notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally, emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
